For Referrers

What types of referral does Shirley accept?

Shirley accept:

- Self-referral

- Private referral from your healthcare provider

- Chronic Disease Management (formerly EPC plan)

- Plan-managed and self-managed NDIS plan with relevant support category

Please note patients will incur an out-of-pocket fee for private and Chronic Disease Management Care Plan (formally EPC) referrals. Medicare rebate is $60.35.  You can arrange your Medicare rebate via Medicare Express phone App or via My Gov website.

How do I refer my clients or patient to Shirley?
 

  • For NDIS referrals:

What are Shirley's consultation fees?
 

Telehealth appointment is charged at:

 

 $ 190 for one-hour initial consultation

 $ 95  for a 30-minutes review

​

All dietitian consultations incur out-of-pocket fees (except NDIS).

Can dietitians sessions cost be claimed through my health fund?

Yes. You will be able to claim session cost with any major private health provider, depending if your level of cover include dietetics services.  Contact your health fund for rebate amount and other details. Dietitian item number are 500 (individual initial consultation) and 600 (individual follow up consultation). You will be provided with an receipt to help make this claim.
